That looks & sounds identical to a #3770262 fan I found awhile ago that fits:
1965-66 series 10-30 trucks w/AC.
1965-68 Impalas w/283 cid.
1964-68 Chevelle-Chevy II w/283 or 307cid.and AC.
1968 Camaros w/S/B.
