When you buy a car from a dealer that is on consignment and are shipping it out of state, how do they handle the title and bill of sale? Does the dealer provide a title with their name on it and a bill of sale from the dealer? If they directly transfer the title from the consingnee, how do you handle the fact that the title is from the owner of car, yet the bill of sale is from the dealer?

Best to contact your DMV and ask exactly what they will require to be able to issue a new title to you. I purchased a car from a California consignor some years ago and he had to take title in his name to complete the sale.
