Yes. I found it in a dealer's file cabinet they were cleaning out.

I'd be interested as well! My car came from Butler Chevrolet in Worcester(32, 111) (a trade from Cargill Chevrolet that NCRS doc lists)

The dealers used it to locate other dealers to do new car swaps, It has the phone number and address for each dealer
