Well, I printed the form way back in Feb and set it aside for "later". Well, it's LATER. Just came across it in a pile of papers and holy cats, the deadline is in a week for pre registration.
Sorry Doug, I'm a master procrastinator. "Check's in the mail" as the saying goes...  I guess this can be a reminder for anyone else coming in just under the wire.
