Yes, believe it is from a Cevrolet Citation .
But they can be fixed. Never had a real one in the last 30 years. I have done 5 of these, just time consuming. Used basically a pr.of right angle plates and a fixture I made to hold the dash. Put a pilot hole in and end milled the slots then hand filed the corners. Patience Young Grasshopper!!
