Does the '69 COPO camaro use the CH code decal, the same as the 69 396/375hp engine or the CN code used for the 427 L72 engine? If the former, any idea why?

Thanks, that's what the MacNeish book said, just seems weird that the L72 sticker would not be used...

Just don't go by what the decal vendor charts say. The decal manufacturer (Osborn) has mistakes in their application listings.
