It's for a small block, but I'm not sure what application:

Your date decode is right.That pump saw very limited useage before being quickly superceded by #3953692.
It would've been originally installed on any SB assorted horse powered 1969 Passenger-Nova-Camaro-Chevelle with 302-307-327-350cid.
